New Hire Paperwork and Documentation (for all new hires)

New employees must be trained within 30 days (or 100 hours)

Workplace Violence Prevention Program (45 minutes)
New Hire Paperwork and Documentation: The following forms are needed to complete your new hire paperwork. Please print and fill out (do not email) and bring documents to LRD, send us an email (assistantmanager@laderarec.org) to let us know when you are coming.
I-9 - for employment verification (note: bring the original documents to your first day of work for compliance purposes)
Employee Information Form and Emergency Contact Information (will be kept confidential for emergency use only)
Work Permit (only necessary for those applicants who are under 18)
CPR and First Aid certification (required for all summer staff - upload a photo of your certification)
Lifeguard certification (if applicable - must be current -upload a photo of your certification)
Returning Employees:
All employees must take harassment prevention training every other year and Workplace Violence Prevention annually. Each training takes one hour.

Board members and managers:
Board members and managers must take both Session A and Session B of harassment prevention training for two full hours every other year and within 6 months of assuming the board member position.

SafeSport Additionally, you will need to take the Safe Sport training once. You will need a code from the LRD manager.
